FAMILIES IN TRANSITION II (FIT II)
Families in Transition II (FIT II) is a comprehensive, intensive clinical case management program that supports and serves young children (birth to six years of age) and their families living in Yonkers and Mt Vernon who are in high-risk mental health situations. As part of Westchester County's System of Care, FIT II provides children and their parents with emotional and practical support as they work to achieve their individual, family and community potential. FIT II is rooted in the belief that young children are best served in their own families and communities.

FIT II provides and integrates clinical services, related case management, home and community-based interventions. The FIT II clinical case management services are individualized and based on the strengths, needs, and culture of each child and their family. Services are provided along a continuum of care and may include:

FIT II's offices are located at the Center for Preventive Psychiatry in Yonkers, New York. As they are funded by Medicaid, third party payments, private foundations and the Westchester County Department of Community Mental Health (DCMH), FIT II services are provided at no-cost to eligible children and their families.
